What would I say to my manager?

Hello! Im about to have nose reshaping surgery in about 2 months time and I need about 3 to 4 weeks off work for my bruises to go, because I am a slow healer.. I dont know what to say to my manager...I dont want everybody to know what I am having done, so should I lie, should I tell her and ask her to keep it a secret or what?
